3. Question Types

Creating a survey using Xebo.ai has never been easier. With a variety of questions available to you, you can pick and choose the type of question which suits your survey the best. 

3.1 Choice

3.1.1 Single Choice

The single choice question type is used when you want  respondents to select one option from the available choices. It is recommended to use this question type only when the number of answer choices is limited. 

The steps to create this question in your survey are as follows: 

Step 1: Click Add Question.

Step 2: Click the Single Choice question type from the right tab of Add New Question.

Step 3: The question will be added to the survey. You can customize its title and options as per your preference.

Step 4: To add multiple options at once, click the Add Bulk Options button. 


Step 5: A popup will appear allowing you to enter multiple options in the form of text. Click Save once you have added all the options.

NOTE: You can also add media files to the question-and-answer choices.

3.1.2 Multiple Choice

Multiple Choice Question type is used when you want your respondents to choose multiple answers from the given answer choices. The steps to adding this question type in your survey are as follows. 

Step 1: Click Add Question on the page you want to populate the question.

Step 2: Click the Multiple Choice question type from the right tab of Add New Question.

Step 3: The question will be added to the survey. You can customize its title and options as per your preference.

Step 4: To add multiple options at once, click the Add Bulk Options button. 

Step 5: A popup will appear allowing you to enter multiple options in the form of text. Click Save once you have added all the required options.

Note: You can also add media files to the question and answer choices.

3.1.3 Dropdown

Drop Down Questions are used when you want the respondents to select one answer choice from a drop-down list of answer choices. This question type serves as a single choice question type, the only difference being that you can use this question type when you have a lot of answer choices and displaying them all at once is not feasible. 

The steps to adding this question type in a survey are as follows:

Step 1: Click Add Question on the page you want to populate the question.

Step 2: Click the Dropdown question type from the right tab of Add New Question.

Step 3: The question will be added to the survey. You can customize its title and options as per your preference.

Step 4: To add multiple options at once, click the Add Bulk Options button. 

Step 5: A popup will appear allowing you to enter multiple options in the form of text. Click Save once you have added all the required options.

NOTE: You can also add media files to the question and answer choices.

3.1.4 Multi Select Dropdown 

Multi Select Drop Down questions are used when you want respondents to select more than one answer choice from a drop-down list. 

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Multi Select Drop Down on the right side of the screen under the New Question tab.

Step 3: The question will be added to the survey. You can customize its title and options as per your preference.

Step 4: To add multiple options at once, click the Add Bulk Options button. 

Step 5: A popup will appear allowing you to enter multiple options in the form of text. Click Save once you have added all the required options.

Step 6: Manage the question settings and insert piping if required.

3.2 Textbox

3.2.1 Single Textbox

Single Textbox question is used when you want your respondents to answer an open-ended short answer. This question type is mostly used when you want to get a quick opinion of respondents. 

The steps to creating this question are as follows:

Step 1: Click Add Question on the page you want to populate the question.

Step 2: Click the Single Textbox question type from the right tab of Add New Question.

Step 3: The question will be added to the survey. You can customize its title and options as per your preference.

Step 4: Insert piping if necessary.

Note: You can add media files to the question. You can also use Validation to ensure that the answers come in a specific pre-defined format. Please refer to Section 4: Question Settings for more information.


3.2.2 Multiple Textbox

Multiple Textboxes question type is used when you want your respondents to answer multiple open-ended, short text questions. You can also validate the question to ensure answers in a specific length or format. 

The advantage of using Multiple Textboxes over multiple Single Textboxes is that it stops you from having excessive questions. If you are asking multiple textbox questions, you can club them under one with Multiple Textboxes question type. 

The steps to add a multiple textboxes question are as follows: 

Step 1: Click Add Question on the page you want to populate the question.



Step 2: Click the Multiple Textbox question type from the right tab of Add New Question.

Step 3: The question will be added to the survey. You can customize its title and options as per your preference.

Step 4: Click the + Add Textbox option if you wish to include more textbox questions inside the Multiple Textbox question.

Step 5: Insert piping if necessary.

Note: You can add media files to the question. You can also use Validation to ensure that the answers come in a specific pre-defined format. Please refer to Section 4: Question Settings for more information.

3.2.3 Text Area

The Text Area question type is used when you want your respondents to answer an open-ended long text answer. This question type is used in a survey when you want a long textual answer from your customers.

The steps to add this question are as follows:

Step 1: Click Add Question on the page you want to populate the question.


Step 2: Click the Text Area question type from the right tab of Add New Question.

 

Step 3: The question will be added to the survey. You can customize its title and options as per your preference.

Step 4: Insert piping if necessary. 

Note: You can add media files to the question. You can also use Validation to ensure that the answers come in a specific pre-defined format. Please refer to Section 4: Question Settings for more information.

3.3 NPS (Net Promoter Score)

The Net Promoter Score (NPS) question is used when you want to measure customer loyalty with a single question. The score represents the net percentage of your customers who are promoters of your company or brand. 

This question is used when you explicitly want to check how many of your customers are your promoters. This is helpful as our platform automatically helps you get a promoter score and the NPS score can also easily be shown in Dashboard using the NPS Widget. 

Based on their response, your customers belong into one of the following categories:

Satisfied but not active promoters who can be wooed by other brands

Promoters

Loyal promoters who are brand advocates and continue to support your brand and recommend it to other users.

3.3.1 Single NPS

The steps to adding an NPS question are as follows:

Step 1: Click Add Question on the page you want to populate the question.

Step 2: Click the Single NPS question type from the right tab of Add New Question.

 

Step 3: Modify the question title, rating labels and colors based on your preference.

3.3.2 Multi NPS

Multi Net Promoter score is an extension of the Net Promoter Score (NPS) question and is used when you want to measure customer loyalty with multiple questions. 

The steps to adding this question type are as follows:

Step 1: Click Add Question on the page you want to populate the question.

Step 2: Click the Multi NPS question type from the right tab of Add New Question.

 

Step 3: Modify the question title, rating labels and colors based on your preference.

Step 4: Insert piping if necessary.

3.3.3 NPS Pro 

NPS Pro questions are used to measure customer loyalty by asking respondents how likely they are to recommend a service or organization on a 0–10 scale. This question type automatically categorizes responses into Promoters, Passives, and Detractors.

Based on this information, each category of users can be shown a set of specific questions. These questions can be customized for each category.

The steps to adding this question type in a survey are as follows:

Step 1: Click Add Question on the page you want to populate the question.

Step 2: Click the NPS Pro question type from the right tab of Add New Question.

Step 3: Modify the question title, rating labels and colors based on your preference. You can modify this for the questions that are shown to each category of users.

Step 4: Review and configure the scale labels if required.

3.4 Matrix

A Matrix question is a closed-ended question type in which respondents are asked to evaluate one or more row items using the same set of column choices.

3.4.1 Rating Matrix

The Rating Matrix question type is used when you want your respondents to rate one or more of your products/brands/services. You can assign weightage to answer choices, and it will be averaged in the analysis part.

The steps to adding rating matrix questions are as follows:

Step 1: Click Add Question on the page you want to populate the question.

Step 2: Click the Rating Matrix question type from the right tab of Add New Question.

Step 3: Modify the question title, row labels, column labels based on your preference. 

Step 4: You can add multiple rows and columns by clicking the icon besides the row and column options.

 

Step 5: Insert piping if necessary. 

3.4.2 Bipolar Matrix

A bipolar matrix is a type of matrix/table question where each row is rated on a scale that runs between two opposite endpoints (bipolar adjectives or labels), and the respondent chooses one point between these extremes for each row.

The steps to adding a Bipolar Matrix question is as follows:

Step 1: Click Add Question on the page you want to populate the question.

Step 2: Click the Bipolar Matrix question type from the right tab of Add New Question.

Step 3: Modify the question title, row labels, column labels based on your preference. 

Step 4: You can add multiple rows and columns by clicking the icon besides the row and column options.

Step 5: Click the Bulk Edit Rows option and type the products/items/services you want to be rated in the Rows section.

Step 6: Click the Bulk Edit Columns option and type the rating scale under the Columns section.

Step 7: Insert piping if necessary. 

3.4.3 Dropdown Matrix

The Matrix Dropdown question type is used when you want respondents to evaluate several items using the same set of measurements by choosing from a preset drop-down list of answer choices. This question type helps you evaluate numerous products/services against their several features/aspects. 

The steps to adding a Matrix Dropdown question are as follows:

Step 1: Click Add Question on the page you want to populate the question.

Step 2: Click the Dropdown Matrix question type from the right tab of Add New Question.


Step 3: Modify the question title, row labels, column labels based on your preference. 

Step 4: For adding a drop-down list to answer choices, hover to the answer choice in the Columns section and click the Add Dropdown Choices button. 

Step 5: You can add multiple rows and columns by clicking the icon besides the row and column options.

Step 6: Insert piping if necessary.

3.4.4 Textbox Matrix

The Matrix Textbox question type is used when you want respondents to give open-ended answers for several products across the set of measurements. Matrix Textbox comes in handy when you want customers’ opinions on multiple products/services across universal parameters. 

The steps to adding this question are as follows:

Step 1: Click Add Question on the page you want to populate the question.

Step 2: Click the Textbox Matrix question type from the right tab of Add New Question.

Step 3: Modify the question title, row labels, column labels based on your preference. 

Step 4: You can add multiple rows and columns by clicking the icon besides the row and column options.

Step 5: Insert piping if necessary. 

3.5 Rating

3.5.1 Single Rating

Rating question type is used when you want your respondents to rate one or more products/brands/services using the same set of measurements. You can also assign weights to answer choice which when viewed during the analysis will show you the weighted average for each answer choice. 

The steps to adding this Question are as follows:  

Step 1: Click Add Question on the page you want to populate the question.

Step 2: Click the Single Rating question type from the right tab of Add New Question.

Step 3: Modify the question title and rating labels based on your preference. 

Step 4: Insert piping if necessary.

3.5.2 Multiple Rating

Multiple Rating questions are used when you want respondents to rate multiple items using the same rating scale. This helps capture feedback on several aspects in a single question.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Multiple Rating on the right side of the screen under rating.

Step 3: On the left side, type the question text.

Step 4: Configure the rating scale (e.g., 3 or 5).


Step 5: Manage the question settings as required.

3.5.3 Rating Pro

Rating Pro questions are used to capture a single rating response using a configurable scale with enhanced visual presentation. Users can be shown a specific set of questions if their rating falls in a specific range.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Rating Pro on the right side of the screen under the rating.

Step 3: On the left side, type the question text.

Step 4: Configure the rating scale range and labels.

Step 5: Select the preferred display style.

Step 6: Manage the question settings if required.

Step 7: Insert piping if necessary.

3.6 Slider

3.6.1 Single Slider

The Slider question type is used when you want the respondents to rate your product/service by dragging a slider on the rating scale.

The steps to adding this question type are as follows:

Step 1: Go to the survey you want to add the question to and click Add Question.


Step 2: Select the single slider question type to add it to the survey.

Step 3: Type the question you want to show to the respondents. Modify the slider labels and question settings as per your requirement.

Step 4: Insert piping if necessary.

3.6.2 Multiple Slider

Multiple Slider questions are used when you want respondents to provide numeric feedback for multiple parameters using slider controls.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Multiple Slider on the right side of the screen under slider.

Step 3: On the left side, type the question text and configure the minimum, maximum and step values for the sliders.

Step 4: Insert piping if necessary.

3.7 File Upload

File Upload question type is used when you want your respondents to upload a media file in the survey. This is useful where you want the user to share images to express their experience/concern/issue.

3.7.1 Drag & Drop

The steps to adding this question are as follows: 

Step 1: Go to the survey you want to add the question to and click the Add Question.

Step 2: Select Drag & Drop on the left side of the screen under the New Question tab.

Step 3: Type the question that you want to show to the respondents.

Step 4: You can add more upload options by clicking + button.


Step 5: Select the file type you want your respondents to upload and the maximum allowed size by clicking File Type and Size.

Step 6: Insert piping if needed.

3.7.2 Button Layout

Button Layout is a File Upload question type that allows respondents to upload files using a button-based interface instead of drag and drop. 

This question type provides two upload options, enabling respondents to either select a file from their device or capture a new image using the device camera.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Button Layout on the right side of the screen under the file upload.

Step 3: On the left side, type the question text.

Step 4: Configure the file upload settings such as allowed file types and maximum file size.

Step 5: Manage the question settings if required and insert piping if necessary.

3.8 Password

The password question type is used when you want to protect your survey using the password. Only the respondents who have the password can access the survey. You can share the password while sharing the survey. You can also use the option of One Time Password (OTP), depending upon your plan.

  • Generic Password

The steps to adding this question type are as follows:

Step 1: Go to the survey you want to add the question to and click the Add Question.

Step 2: Select Password on the right side of the screen under the Add New Question tab.

Step 3: Type the question you want to show to the respondents and enter password choices.

Step 4: Insert piping if necessary.

3.8.2 OTP

OTP (One-Time Password) is used to verify respondent identity before allowing access to the survey, ensuring secure and authenticated responses.

The steps to enabling OTP for a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select OTP as the Question Type.


Step 3: On the left side, type the question text.



Step 4: Select the required Gateway from the drop-down list to manage OTP delivery and validation.


Step 5: Insert piping if necessary.

3.9 Statement

3.9.1 Paragraph

The statement question type is used when you want to convey a message or state a fact/guideline for your respondents. The advantage of this is that you can place it anywhere in the survey, like other question types.

The steps to add a Statement are as follows:

Step 1: Go to the survey you want to add the question to and click the Add Question.

Step 2: Select Statement on the left side of the screen under the New Question tab.

Step 3: Type the statement on the top left side of the screen.

4. Manage the question setting and insert piping if needed.

3.9.2 Header & Paragraph

Header & Paragraph is a statement-type component used to display informational content within a survey. It allows survey creators to add a heading and descriptive text to introduce the survey, provide instructions, or explain a section.

The steps to adding this component in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the content.


Step 2: Select Header & Paragraph on the right side of the screen under the Statement section.

Step 3: Enter the Paragraph text to provide instructions or descriptive information.

3.10 Ordering

3.10.1 Rank Order

Rank Order questions are used when you want respondents to prioritize or rank a list of options based on preference or importance. 

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Rank Order on the right side of the screen under the Ordering section.

Step 3: On the left side, type the question text.

Step 4: Add the bulk options that respondents need to rank.

3.10.2 Drag and Rank

Drag and Rank questions allow respondents to rank items by dragging and dropping them into their preferred order. This provides an interactive way to capture prioritization.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Drag and Rank on the right side of the screen under the Ordering section.

Step 3: On the left side, type the question text.

Step 4: Add the list of options to be ranked manually or use bulk option.

Step 5: Add minimum and maximum rank limit.

3.10.3 Click and Rank

Click and Rank questions enable respondents to assign ranks by clicking options rather than dragging them. 

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Click and Rank on the right side of the screen under the Ordering section.

Step 3: On the left side, type the question text.

Step 4: Add the options to be ranked manually or use bulk option.

3.10.4 Constant Sum

Constant Sum questions are used when you want respondents to distribute a fixed total value (such as 100 points) across multiple options based on importance or preference.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Constant Sum on the right side of the screen under the Ordering section.

Step 3: On the left side, type the question text.

Step 4: Add the items to be evaluated.

Step 5: Define the total value to be distributed (e.g., 100).

Step 6: Manage validation rules to ensure the total is correctly allocated.

3.10.5 Swipe Card

A swipe card question is an interactive, mobile‑friendly format where each option is shown as a card that the respondent swipes left/right (or similar) to answer.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Swipe Card on the right side of the screen under the Ordering section.

Step 3: Modify the question text as per your preference.

Step 4: Modify the list of cards that show by clicking the Edit Cards button.

Step 5: Customize the order of cards and maximum cards to show under the Settings menu.

3.10.6 Card Sort

Card Sort questions are used to understand how respondents group or categorize items. Respondents sort cards into predefined or custom categories, helping identify logical groupings.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Card Sort on the right side of the screen under the Ordering section.

Step 3: On the left side, type the question text.

Step 4: Add the cards list and categories list.

Step 5: Manage the order of cards and maximum cards to show for each respondent under the Settings menu.

3.11 Interactive Feedback

3.11.1 Audio Feedback

Audio Feedback questions allow respondents to record and submit voice responses directly within the survey. This question type is useful when capturing detailed verbal feedback instead of typed responses.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.



Step 3: On the left side, type the question text.

Step 4: Add the cards list and categories list.

Step 5: Manage the order of cards and maximum cards to show for each respondent under the Settings menu.

3.11 Interactive Feedback

3.11.1 Audio Feedback

Audio Feedback questions allow respondents to record and submit voice responses directly within the survey. This question type is useful when capturing detailed verbal feedback instead of typed responses.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Audio Feedback on the right side of the screen under the Interactive Feedback section.

Step 3: On the left side, type the question text.

Step 4: Manage the question settings as required and insert piping if necessary.

3.11.2 Video Feedback

Video Feedback questions allow respondents to record and upload video responses using their device camera. This helps capture facial expressions and detailed explanations.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Video Feedback on the right side of the screen under the Interactive Feedback section.

Step 3: On the left side, type the question text.

Step 4: Manage the question settings as required and insert piping if necessary.

3.11.3 Heat Map

Heat Map questions allow respondents to click specific areas of an image to indicate focus, interest, or feedback. The collected clicks are visually represented as heat zones for analysis.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Heat Map on the right side of the screen under the Interactive Feedback section.

Step 3: On the left side, type the question text.

Step 4: Click the Browse button to upload a image to be used in the Heatmap question.

Step 5: Select image and click Save to be added in the heatmap question.

Step 6: Configure the number of maximum areas and minimum areas the respondent can highlight under the Settings menu.

3.11.4 Hot Spot

Hot Spot questions allow respondents to select specific marked areas on an image. This is useful for identifying exact points of interest or concern.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Hot Spot on the right side of the screen under the Interactive Feedback section.

Step 3: On the left side, type the question text.

Step 4: Upload the image using the Browse button and selecting or uploading an image.

Step 5: Define the selection areas which user can select for rating the areas.

2.11.5 Audio Video Feedback

Audio Video Feedback questions allow respondents to record both audio and video responses within the survey. This provides a richer and more expressive form of feedback.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Audio Video Feedback on the right side of the screen under the Interactive Feedback section.

Step 3: On the left side, type the question text.

Step 4: Manage the question settings as required and insert piping if necessary.

3.12 Price Sensitivity 

3.12.1 Van Westendorp Textbox

Van Westendorp Textbox is a price sensitivity question type used to understand how respondents perceive pricing by collecting direct numeric inputs through text fields. It helps determine acceptable price ranges and perceived value.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Van Westendorp Textbox on the right side of the screen under the Price Sensitivity section.

Step 3: On the left side, type the question text.

Step 4: Configure the minimum and maximum price range a respondent can specify under the Validation settings.

3.12.2 Van Westendorp Dropdown

Van Westendorp Dropdown is a price sensitivity question type that allows respondents to select price perceptions using predefined dropdown options instead of manual entry.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Van Westendorp Dropdown on the right side of the screen under the Price Sensitivity section.

Step 3: On the left side, type the question text.

Step 4: Add dropdown price options for respondents to select from by clicking the Bulk Edit button.

Step 5: Modify the answers and click Save to modify value options in the dropdown.

3.12.3 Gabor Granger

Gabor Granger is a price testing method used to evaluate how likely respondents are to purchase a product or service at different price points.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Gabor Granger on the right side of the screen under the Price Sensitivity section.

Step 3: On the left side, type the question text.

Step 4: Configure the different price points to be tested and shown to the respondent. The price points should be arranged in an ascending order. 

You can add multiple price points by clicking the + Add Option icon.

Step 5: Configure the currency, starting price, price path and setup via the Settings menu on the right hand side.

Step 6: Manage the question settings and add piping if necessary.

3.13 Usability Testing

3.13.1 Figma

Figma is a usability testing question type used to embed or test design Figma prototypes within the survey to collect feedback on user experience and interface design.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Figma on the right side of the screen under the Usability Testing section. 

Step 3: Modify the question text as per your preference.

Step 4: Insert the Figma prototype link to be shown to the respondent in the survey.

3.14 Maps 

3.14.1 Maps

Maps question type allows respondents to interact with a map interface to provide location-based feedback or mark areas of interest.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Maps on the right side of the screen under the Maps section. 

Step 3: On the left side, type the question text.

Step 4: Select the country for which the map needs to be displayed.

Step 5: Manage the question settings and apply piping if necessary.

3.14.2 Location

Location question type is used to capture the respondent’s geographic location through map selection or location input.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select Location on the right side of the screen under the Maps section. 

Step 3: On the left side, type the question text.

Step 4: Manage the question settings as needed and insert piping if necessary.

3.15 Choice Modelling 

3.15.1 MaxDiff

MaxDiff (Maximum Difference Scaling) is a choice modelling question type used to identify preferences by asking respondents to select the most and least important options from a set.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question.

Step 2: Select MaxDiff on the right side of the screen under the Choice Modelling section. 

Step 3: On the left side, type the question text.

Step 4: Add the list of attributes or options to be evaluated. These can be products or services for which you want the respondent to mark their importance against each other.

You can add additional attributes by clicking the + Add Option button or by clicking the Add Bulk Options button.

Step 5: Configure the maximum number of attributes that that should be visible in a MaxDiff study, the number of attributes visible in a single task / step and how many attributes should repeat across multiple tasks / steps. 

NOTE: Product of Max Attributes and Repeat Attributes should be divisible by Attributes per Task.

3.16 Miscellaneous 

3.16.1 Captcha

CAPTCHA (Completely Automated Public Turing test to tell Computers and Humans Apart) is used to verify that the respondent is a real user and prevent automated or bot submissions. It is typically added at the starting of a survey to verify user early on.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question. This should be the first page and the first question of the survey.

Step 2: Select Captcha on the right side of the screen under the Miscellaneous section. 

Step 3: Customize the question text as per your preference. 

3.16.2 Contact

Contact question type is used to collect respondent contact details such as name, email, or phone number.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question. 

Step 2: Select Contact on the right side of the screen under the Miscellaneous section. 

Step 3: Select which contact fields to include in the question by selecting or deselecting options from the Default Options list.

You may add custom contact fields by clicking the + Add Option icon.

Step 4: Configure the question text and add piping if necessary.

3.16.3 Date & Time

Date & Time question type allows respondents to select a specific date, time, or both using a calendar and time picker.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question. 

Step 2: Select Date & Time on the right side of the screen under the Miscellaneous section. 

Step 3: On the left side, type the question text.

Step 4: Configure whether date and time should be visible and configure the calendar type via the Question settings.

Step 5: Configure the question text and add piping if necessary.

3.16.4 Survey Forward

Survey Forward allows respondents to forward or share the survey with other participants, helping increase reach and participation. The survey forward question can be added at the end of a block, allowing other participants to continue in their survey.

The steps to adding this question type in a survey are as follows:

Step 1: Click the Add Question button on the page where you want to add the question. This should be the last question of the survey.

Step 2: Select Survey Forward on the right side of the screen under the Miscellaneous section. 

Step 3: Customize the question text and email text to be sent with the survey forward link.

Step 4: Configure the question text and add piping if necessary.
