2. Guide to Creating Surveys

2.1 Creating a New Survey

To create a new survey, click the Create Survey button on the top right corner of your account page.



2.1.1 Start from Scratch

  1. Select Start From Scratch to initiate the creation of a new survey template and configure it according to your requirements.

  1. Enter the survey name in the Survey Title field, select the survey category that best fits your survey for proper classification, and then choose the folder in which you want the survey to be stored.


  1. Click Start Creating to proceed with building your survey.

2.1.2 Copy Another Survey

  1. Locate and select the relevant survey, then click the three dots at the far right to access additional options.

  1. Click Create a Copy to create a new survey using the selected template or existing survey as a starting point. With this option, you have the choice to use the questions from another existing survey in your library.


2.1.3 Create from Existing Template

  1. Browse through the available templates and click the one that best aligns with your survey’s purpose.

Note: With this option, you can start your survey quickly using templates designed by our experts. Scroll down to see the ready-to-use survey templates available to you. You can later customize the questions, layout, and design according to your needs.

2.1.4 Import from File (Word)

The Import Using File (Word) feature is designed specifically for scenarios where designers often receive surveys from internal teams, stakeholders or methodology team in the form of a word document. To access this feature:

Step 1: Click the Import from File (Word) option under Create a Survey tab.

Step 2: Click Browse Files or drag and drop your file into the upload area. Supported formats include DOCX, XLSX, CSV, and PDF (maximum file size: 10 MB).

Step 3: After the file is uploaded successfully, click Next and enter the required survey details, such as the survey name and other configuration settings.

Step 4: Complete the setup process and create the survey. Review the imported questions, logic, and settings before publishing or sharing the survey.

2.2 Adding Questions to Your Survey

  • Adding Questions Manually

Step 1: Click the Add Question button on the survey builder. 

Step 2: Select from the list of question types available. You may use the Search functionality to find a specific question type that caters as per your requirement. You have the following options on this screen:

  • Build your Own – Use any of the available question types from scratch and customize question title, options and logics.
  • Select from Library – Use the ADRD library or select from questions added by your organization in the Question Library

  • Deleted Questions – If you have deleted any questions from the current survey previously, they will be displayed in this tab.

2.2.2 Using OneBot for Adding Questions

The OneBot AI Assistant helps users design structured surveys using natural language prompts. It accelerates survey creation while ensuring methodological accuracy and consistency. 

To access the AI survey creation assistant:

Step 1: On the survey creation screen, from the bottom right corner, select OneBot AI Assistant. 

Step 2: The AI prompt panel will open, allowing you to describe your survey objective. (Maximize the OneBot AI Assistant Window to access previous chats.) 

When creating complex surveys, prompts should clearly describe: 

  • The purpose of the survey 
  • The target audience 
  • The metrics required 

Example Prompt (Simple) 

“Create a customer satisfaction survey with NPS and one open-ended question.” 

Example Prompt (Complex) 

“Create a post-service survey for government service users that includes NPS, CSAT, effort score, and logic-based follow-up questions for detractors. Include one open-ended question to capture improvement suggestions.” 

Example Prompt (Advanced) 

“Design a multi-stage citizen feedback survey for a digital service journey. Include NPS, service clarity rating, effort score, and conditional follow-up questions based on low scores. Add one open-ended question for service improvement and structure the survey for email and mobile delivery.” 

Step 3: Once the prompt is submitted, the OneBot AI Assistant generates a complete survey structure, and a list of prompt result questions will be displayed in the OneBot AI Assistant Window.

Step 4: Select the generated questions as per need/objective, then click “Add question for me”. Questions will be added to the survey builder. 

NOTE: All AI-generated content is fully editable. You can: add or remove questions, change question types (e.g., CSAT to CES), edit wording or scale labels and add logic rules manually.

2.3 Applying Logic

After you have created a survey and added questions, it’s time for you to apply logic to the survey.

Step 1: Click the Logic icon to add a logic to the selected question.

Step 2: Choose from the list of available logics to further customize your survey. You may include multiple logics within a single survey question.

2.4 Customize the Design

Once your survey is ready with all the questions and logic, the next step is to make it more pleasant using the wide range of theme options to choose from. 

Step 1: Click the Theme button and you can choose from the pre-existing themes.

Step 2: Once you have selected the theme, you can either choose the default settings of the selected theme or you can customize your theme.

Click the Create Theme button to create additional themes.

2.5 Preview & Test your Survey

Once you have fine-tuned everything, it’s time to preview the survey. To preview your survey,

Step 1: Click the Preview button on the top right corner of your screen. You can preview an entire survey, a specific question or a block of questions.

Step 2: A separate tab will open with the survey. Click Preview to view the survey in both laptop and mobile versions, and click Close to exit the tab once you are satisfied with the look and feel.

To share the test link with someone, click the test link next to the eye icon and copy the link from there and then you can share it further.

NOTE: The Test link functionality shows the latest published version of the survey.

2.6 Publish the Survey

Publishing feature is a new addition to the survey creation process.. 

Previously, users could directly make changes to a survey, and those changes are immediately reflected in the live version. Now, users must publish the survey to make changes permanent in the live survey.

Any changes made to the survey are now saved as a draft. To make these changes permanent, users must publish the survey. However, the Publish button remains inactive if there are no changes to publish. Once you make changes to your survey, the Publish button becomes active.

With the addition of the publish button, users also have the option to check the publishing history, which displays details of every instance when changes were made live to the survey. The publishing history page shows the following details:

  • Name of the user who published the survey 
  • Version info
  • IP address of the device from which the changes were made  
  • Publish date and time

Under the Publish History section, users can track all instances when the survey was published. 

Step 1: Click the button beside the test link button and click Publish History.

Step 2: A popup will appear displaying the previous survey versions along with information related to each survey version.

  • Published By: This shows who published the survey. Users are able to see the email of the person who published the changes.
  • Version: This shows the version of the survey that was published. Every time a new change is saved, it becomes a new version.
  • Device Info: This shows the IP address of the device from which the changes were made.
  • Published Date: This shows the date and time when the changes were published.


Step 3: If in case user wishes to restore to a prior version of the survey, they must click the Restore button beside the survey version. 



All the features combined allow users to have more control over an active survey, ensuring that every change made on a live survey is properly recorded, and users can return to the previous version with a single click.
